What
do Case Managers do?
- Advocate via many activities
for the health and well being of the client.
- Develop a comprehensive
care plan.
- Act as liaison to families
- ensure things are going well and alert family to problems.
- Conference and counsel
with families.
- Coordinate basic financial
activities.
- Provide information and
recommendations regarding community and social services.
- Oversee personal care.
- Interact with physicians,
home health agencies, and other health care professionals to provide continuity
of care.

What
do Caregivers do?
- Provide supportive,
hands-on personal care including dressing, bathing and toileting assistance.
- Give Reminder for
self-administered medications.
- Help with mobility: walking
and transfers.
- Prepare meals and provide
eating assistance.
- Perform light housekeeping
as part of other personal services provided.
- Run errands, help with
shopping or do the shopping.
- Provide friendly, caring
companionship or supervision and guidance.

INFORMATION
YOU SHOULD HAVE AVAILABLE AND QUESTIONS YOU SHOULD BE PREPARED TO ANSWER WHEN
REQUESTING SERVICE FROM A HOME CARE PROVIDER:
- It is important to be
up-front and honest about the physical and mental limitations of you or your
family member.
- Who is the primary contact
person? When is the best time to reach them? What contacts should be made
in case of an emergency situation?
- Who are the other professionals
involved with this client? Include the name of the person's doctor, the skilled
care agency, physical therapist, etc.
- What is the physical
and mental status of the client? Is he/she incontinent, on medications, alert,
ambulatory?
- How much service will
be needed (the number of days per week and the hours per day)?
- Who will pay for the
service? If you are covered by a long-term care insurance policy that will
pay for this type of service, you will need the name of the insurance company
and the client's policy number.
