Comprehensive Case Management is a key to providing coordinated activities of daily living through the efforts of a Case Manager. The goal is to develop a plan of care that allows our clients to live in the least restrictive and safest environment while remaining at home. Maintaining optimum independence and dignity is always the goal. The large and very capable staff at Home Care and Elder Services, Inc. includes an experienced Social Worker, RNs, Certified Nursing Assistant's, Companions and Homemakers. All of our personnel are thoroughly screened and trained before they are assigned to a case.
